INVESTIGATORS TRYING to explain the horrible elevator accident that killed ad exec Suzanne Hart are looking at whether a power surge made the lift go haywire, sources say. A sudden surge in an elevator’s power source could override its safety systems and send it shooting up, said a source familiar with the investigation. City Buildings Department officials have said they’re looking at maintenance performed on the elevator’s electrical system just hours before Hart stepped inside. Three sources familiar with the probe said Transel Elevator, the company that maintains the 13 elevators at 285 Madison Ave., replaced a circuit board in the lift shortly before the accident. The sources include two with direct knowledge of the investigation and one in the elevator industry. Witnesses say Hart stepped into the elevator on her way to work at 10 a.m. on Wednesday.  The door didn’t close, and the elevator shot straight up, trapping her lower body between the cab and the wall. The elevator has a number of redundant safety devices. The doors rely on motion sensors that indicate a passenger is entering and prevent the door from closing. And an electromechanical interlock is designed to keep he cab from moving until the doors are completely closed. In this case, because witnesses say the door did not close before the cab moved, sources say it indicates the motion sensor and the interlocking device failed. Among other areas of inquiry, investigators are trying to determine whether the circuit board replacement contributed to a sudden power surge that overrode those safety devices just as Hart entered the cab, according to the two sources with direct knowledge. The industry source described such a surge as “generator suicide,” referring to the elevator power source. The source said such surges typically cause little harm because they occur when doors are closed and result in passengers experiencing a sudden upward jerking motion. If such a surge occurred just as Hart entered the elevator, the source said, the timing “was a billion to one. It’s incredible.” On Friday, the three agencies looking at the accident — the city Buildings Department, the city Department of Investigation, and the U.S. Occupational Safety and Health Administration — declined comment.

Nope, nothing to see here folks, just move along. I mean all that happened was a billion to one chance of happening accident that accrued do to a faulty circuit board that just so happened to be installed one hour before the guy entered the elevator, that's all.

Suzanne Hart, the advertising ace killed Wednesday in a freak elevator accident, had success in her genes.
 
The 41-year-old daughter of a hard-charging executive who was once president of MasterCard International, Hart forged her own path through the business world with a mixture of wit, charm — and hard work.
 
Hart was, in the words of a colleague, a “crazy hard worker” who traveled often for her job and regularly pulled all-nighters on deadline.
 
“Most people who worked with her would tell you that she was one of the nicest people around,” the colleague said, adding that Hart was known for her “big smiles and a quick wit.”
 
A California girl, Hart attended Palos Verdes High School where she made a deep impression on classmates.
 
“She was so sweet and loving a person and her family have always been amazingly kind and generous to all who knew them,” recalled Andy Marckesano.
 
Hart graduated from Knox College in Illinois in 1993 with degrees in fine art and international relations and — inspired by her dad Alex — headed to Manhattan where she landed a job with the Merkley and Partners ad agency.
 
By June 2007, she was at the Young and Rubicam agency where she was director of new business content.
 
Hart’s job was to drum up new business and prepare pitches for new clients.

Do you think that it could been she was killed as a warning over something her old man did?

The only other thing that comes to my mind, and this is grasping at less then straws here, is that she might of either over heard or came across something she wasn't suppose to. But either way, given the data digging skill a lot of the guys have here it should only be a matter of time before someone turns up something.

Redundancy in safety mechanisms is the hallmark of elevator construction and maintenance. As someone who grew up in a family that is in the elevator construction and maintenance business, I can tell you that safety issues are what elevator people talk about ALL the time. It's their biggest concern. So a catastrophic failure of this kind will get a lot of attention from people in this business; and it will be interesting to see what they say as more facts come out. (IF more facts are allowed to come out).

2.26.3 - "Where electromechanical contactors or relays are provided to fulfill the requirements of 2.26.8.2, and 2.26.9.3 through 2.26.9.7, they shall be considered to be used in critical operating circuits. If contact(s) on these electromechanical contactors or relays are used for monitoring purposes, they shall be prevented from changing state if the contact(s) utilized in a critical operating circuit fail to open in the intended manner. The ability of the monitoring contact(s) to perform this function shall not be solely dependent upon springs."

A critical operating circuit is defined by 2.26.3 as those circuits, operations, and devices mentioned in 2.26.8.2 and 2.26.9.3 through 2.26.9.7, which use relays, switches, or contactors for monitoring purposes. The monitoring determines whether a critical contact has remained in the "On" or "Closed" position when it should be "Off" or "Open."

A failure in the brake circuit (2.26.8.2)

A failure that would render any electrical protective devices to be ineffective (2.26.9.3(a))

A failure that would allow the car to move out of the leveling zone with a car or hall door open (2.26.9.3(b))

A failure permitting excessive speeds during access, inspection, or leveling operation (2.26.9.3(c))

A failure which would permit normal operation when on access, inspection, or bypass operation (2.26.9.3(d))

A failure that would continue to make ineffective any door or gate interlock or contact when either a bypass or access switch is in the "OFF" position (2.26.9.3(e))

A failure in the devices controlling elevators with driving motors employing static control without motor generator sets (usually DC motors) (2.26.9.5)

A failure in the devices controlling elevators employing alternating current driving motors driven from a direct current power source through a static inverter (always AC motors) (2.26.9.6)

A failure in the "suicide circuit" of an elevator employing generator field control (always DC motors) (2.26.9.7)

Relays, switches, and contactors in these circuits must monitor their own status. You may have noticed that many of the critical relays, switches, and contactors on electric elevators conforming to the A17.1 - 2000 code and later editions drop-out after each run. (Even the safety chain relay which is traditionally always "Up" on vintage elevators drops-out after each run of the car.) A circuit similar to the following example will be found somewhere in the wiring diagram. If the processor does not receive the signal (ACO) that all the devices have dropped-out the car can not run again.

The types of relays, switches, and contactors used in these critical circuits have several names. They might be called force-guided, positively guided, captive contact, or safety relays. The main difference between these relays and "regular" relays is that the normally open and normally closed contacts must be mechanically linked so that both can not be closed at the same time. The following image depicts two sets of contacts arranged in a force-guided configuration. (see source link for images)

The open doors would have represented a failed contact: the elevator should NOT have run.
In the year 2000, the language in the code governing elevator controller systems was changed:

Language was added to the 2000 code that requires controller manufacturers to monitor for a "software system failure". Simply driving a standard relay with a microprocessor output, which was standard practice on pre-2000 products (refer to Figure 3 - CSAF output), is not sufficient. The software must be separate and independent of the software system. If the software monitor drives a relay, it must be a force-guided relay that is monitored at least once per operating cycle.

Software System Monitor
A further requirement of the new code states that the completed controller will be subjected to high levels of radio frequency interference (RFI). This testing must be performed by an independent testing laboratory and needs to prove that the elevator responds in a safe manner to such levels of RFI as specified by the code.

In summary, when replacing the function of an electrical protective device with solid-state devices, relays or software systems, the circuit used to replace the EPD must be designed so that a single failure of of the components does not compromise safety. Second, the resultant critical operatring circuiit must be monitored os that the first failure of any component is detected and further operation of the car is prevented until the failure is corrected. Third, failure of the software system must not in any way compromise safety. Fourth, all critical operation circuits must be checked on ce per operating cycle. Finally, high levels of FRI must not compromise the safety of the complete controller. If any of these steps has been neglected, the resultant control system does not comply with ASME A17.1-2000/B44-00 Safety Code For Elevators and Escalators.

A Brooklyn woman was burned alive Saturday when her demonic ex-beau ambushed her inside an elevator, sprayed her with a flammable liquid, set her ablaze and tossed a Molotov cocktail inside, cops and neighbors said.

Early Sunday, police were questioning a suspect who turned himself in at Brooklyn’s Transit District 32, stinking of gasoline.

Delores Gillespie, 64, was on her way home from buying groceries when she stepped out of the elevator at 203 Underhill Ave. in Prospect Heights and into a carefully laid trap.

Minutes earlier, Gillespie’s attacker had glued shut the locks on the elderly woman’s fifth-floor apartment and doused the elevator door with accelerant, sources said.

When the elevator reached her floor about 4:15 p.m., she was met by the psycho clad in black and clutching a barbecue lighter, a wine bottle and a canister of combustible fluid resembling an exterminator’s kit.

He barged into the elevator and sprayed her with a mist of accelerant from point-blank range. Gillespie turned from her attacker, crouched down and threw her hands up to her face, grocery bags dangling from her arms.

The assailant cornered her and started spraying her methodically over her face and torso, police said.

Then, the madman, wearing white gloves and a dust mask on his head, lit a Molotov cocktail, waited for a few seconds and set his ex on fire with the wick.

“She goes up in flames and falls to the floor,” NYPD spokesman Paul Browne said.

The maniac wasn’t done.
